This issue happens very coincidentally; however adding mdelay(1) in
mmc_wait_data_done as below could duplicate it easily.

At runtime, IRQ or ICache line missing may just happen at the same place
of the mdelay(1).
This patch gets the mmc_context_info at the beginning of function, it can
avoid this race condition.

The current handler of MMC_BLK_CMD_ERR in mmc_blk_issue_rw_rq function
may cause new coming request permanent missing when the ongoing
request (previoulsy started) complete end.
The problem scenario is as follows:
(1) Request A is ongoing;
(2) Request B arrived, and finally mmc_blk_issue_rw_rq() is called;
(3) Request A encounters the MMC_BLK_CMD_ERR error;
(4) In the error handling of MMC_BLK_CMD_ERR, suppose mmc_blk_cmd_err()
end request A completed and return zero. Continue the error handling,
suppose mmc_blk_reset() reset device success;
(5) Continue the execution, while loop completed because variable ret
is zero now;
(6) Finally, mmc_blk_issue_rw_rq() return without processing request B.
The process related to the missing request may wait that IO request
complete forever, possibly crashing the application or hanging the system.
Fix this issue by starting new request when reset success.

In commit 5491ce3f79ee ("mmc: sdhci-pxav3: add support for the Armada
38x SDHCI controller"), the sdhci-pxav3 driver was extended to include
support for the SDHCI controller found in the Armada 38x
processor. This mainly involved adding some MBus window related
configuration.
However, this configuration is currently done too early in ->probe():
it is done before clocks are enabled, while this configuration
involves touching the registers of the controller, which will hang the
SoC if the clock is disabled. It wasn't noticed until now because the
bootloader typically leaves gatable clocks enabled, but in situations
where we have a deferred probe (due to a CD GPIO that cannot be taken,
for example), then the probe will be re-tried later, after a clock
disable has been done in the exit path of the failed probe attempt of
the device. This second probe() will hang the system due to the clock
being disabled.
This can for example be produced on Armada 385 GP, which has a CD GPIO
connected to an I2C PCA9555. If the driver for the PCA9555 is not
compiled into the kernel, then we will have the following sequence of
events:
1. The SDHCI probes
2. It does the MBus configuration (which works, because the clock is
left enabled by the bootloader)
3. It enables the clock
4. It tries to get the CD GPIO, which fails due to the driver being
missing, so -EPROBE_DEFER is returned.
5. Before returning -EPROBE_DEFER, the driver cleans up what was
done, which includes disabling the clock.
6. Later on, the SDHCI probe is tried again.
7. It does the MBus configuration, which hangs because the clock is
no longer enabled.
This commit does the obvious fix of doing the MBus configuration after
the clock has been enabled by the driver.

f9fd54f22e ("mmc: sh_mmcif: Use msecs_to_jiffies() for host->timeout")
changed the timeout value from 1000 jiffies to 1s. In the case where
HZ is 1000 the values are the same. However, for smaller HZ values the
timeout is now smaller, 1s instead of 10s in the case of HZ=100.
Since the timeout occurs in spite of a normal data transfer a timeout of
10s seems more appropriate. This restores the previous timeout in the
case where HZ=100 and results in an increase over the previous timeout
for larger values of HZ.

The PM_RESTORE_PREPARE is not handled now in mmc_pm_notify(),
as result mmc_rescan() could be scheduled and executed at
late hibernation restore stages when MMC device is suspended
already - which, in turn, will lead to system crash on TI dra7-evm board:

During kernel boot, it will try to read some logical sectors
of each block device node for the possible partition table.
But since RPMB partition is special and can not be accessed
by normal eMMC read / write CMDs, it will cause below error

The reset control for the sunxi mmc controller is optional. Some
newer platforms (sun6i, sun8i, sun9i) have it, while older ones
(sun4i, sun5i, sun7i) don't.
Use the properly stubbed _optional version so the driver does not
fail to compile when RESET_CONTROLLER=n.
This patch also adds a check for deferred probing on the reset
control.

According to erratum 'ERR-7878951' Armada 38x SDHCI controller has
different capabilities than the ones shown in its registers:
- it doesn't support the voltage switching: it can work either with
3.3V or 1.8V supply
- it doesn't support the SDR104 mode
- SDR50 mode doesn't need tuning
The SDHCI_QUIRK_MISSING_CAPS quirk is used for updating the
capabilities accordingly.

According to erratum 'FE-2946959' both SDR50 and DDR50 modes require
specific clock adjustments in SDIO3 Configuration register. However,
this register was not part of the device tree binding. Even if the
binding can (and will) be extended we still need handling the case
where this register was not available. In this case we use the
SDHCI_QUIRK_MISSING_CAPS quirk remove them from the capabilities.

Current code checks "clk_delay_cycles > 0" to know whether the optional
"mrvl,clk_delay_cycles" is set or not. But of_property_read_u32() doesn't
touch clk_delay_cycles if the property is not set. And type of
clk_delay_cycles is u32, so we may always set pdata->clk_delay_cycles as a
random value.
This patch fix this problem by check the return value of of_property_read_u32()
to know whether the optional clk-delay-cycles is set or not.

Commit 0dcaa2499b7d ("sdhci-pxav3: Fix runtime PM initialization") tries
to fix one hang issue caused by calling sdhci_add_host() on a suspended
device. The fix enables the clock twice, once by clk_prepare_enable() and
another by pm_runtime_get_sync(), meaning that the clock will never be
gated at runtime PM suspend. I observed the power consumption regression on
Marvell BG2Q SoCs.
In fact, the fix is not correct. There still be a very small window
during which a runtime suspend might somehow occur after pm_runtime_enable()
but before pm_runtime_get_sync().
This patch fixes all of the two problems by just incrementing the usage
counter before pm_runtime_enable(). It also adjust the order of disabling
runtime pm and storing the usage count in the error path to handle clock
gating properly.

Host controllers lacking the required internal vmmc regulator may still
follow the spec with regard to the LSB of SDHCI_POWER_CONTROL. Set the
SDHCI_POWER_ON bit when vmmc is enabled to encourage the controller to
to drive CMD, DAT, SDCLK.
This fixes a regression observed on some Qualcomm and Nvidia boards
caused by 5222161 mmc: sdhci: Improve external VDD regulator support.

Sleep in atomic context happened on Trats2 board after inserting or
removing SD card because mmc_gpio_get_cd() was called under spin lock.
Fix this by moving card detection earlier, before acquiring spin lock.
The mmc_gpio_get_cd() call does not have to be protected by spin lock
because it does not access any sdhci internal data.
The sdhci_do_get_cd() call access host flags (SDHCI_DEVICE_DEAD). After
moving it out side of spin lock it could theoretically race with driver
removal but still there is no actual protection against manual card
eject.

Commit f1d2736c8156 (mmc: dw_mmc: control card read threshold) added
dw_mci_ctrl_rd_thld() with an unconditional write to the CDTHRCTL
register at offset 0x100. However before version 240a, the FIFO region
started at 0x100, so the write messes with the FIFO and completely
breaks the driver.
If the version id < 240A, return early from dw_mci_ctl_rd_thld() so as
not to hit this problem.

Since commit 89168b489915 ("mmc: core: restore detect line inversion
semantics"), the SD card on i.MX28 (and possibly other) devices isn't
detected and booting stops at:
[ 4.120617] Waiting for root device /dev/mmcblk0p3...
This is caused by the MMC_CAP2_CD_ACTIVE_HIGH flag being set incorrectly
when the host controller doesn't use a GPIO for card detection (but
instead uses a dedicated pin). In this case mmc_gpiod_request_cd() will
return before assigning to the gpio_invert variable, leaving the
variable uninitialized. The variable then gets used to set the flag.
This patch fixes the issue by making sure gpio_invert is set to false
when a GPIO isn't used. After this patch, i.MX28 boots fine.
The MMC_CAP2_RO_ACTIVE_HIGH (write protect) flag is also set incorrectly
for the exact same reason (it uses the same uninitialized variable), so
this patch fixes that too.

Braswell eMMC host controller specifies an incorrect
timeout clock frequncy in the capabilities registers.
The correct value is 1 MHz. A similar fix was done
for sdhci-pci, however in the sdhci-acpi case the
HID/UID is not unique so the capabilities register
values are matched also.

Clear QUEUE_FLAG_ADD_RANDOM in all block drivers that set
QUEUE_FLAG_NONROT.
Historically, all block devices have automatically made entropy
contributions. But as previously stated in commit e2e1a148 ("block: add
sysfs knob for turning off disk entropy contributions"):
- On SSD disks, the completion times aren't as random as they
are for rotational drives. So it's questionable whether they
should contribute to the random pool in the first place.
- Calling add_disk_randomness() has a lot of overhead.
There are more reliable sources for randomness than non-rotational block
devices. From a security perspective it is better to err on the side of
caution than to allow entropy contributions from unreliable "random"
sources.

it uncovers an error where the variable is used by non-init data structures.

"mmc: host: switch OF parser to use gpio descriptors"
switched the semantic behaviour of card detect and read
only flags such that the inversion capability flag would
only be set if inversion was explicitly specified in the
device tree, in the hopes that no-one was using double
inversion.
It turns out that the XOR:ing between the explicit
inversion was indeed in use, so we need to restore the
old semantics where both ways of inversion are checked
and the end result XOR:ed.

The commit 46420dd73b80 (PM / Domains: Add APIs to attach/detach a PM
domain for a device) started using errno values in pm.h header file.
It also failed to include the header for these, thus it caused
compiler errors.
Instead of including the errno header to pm.h, let's move the functions
to pm_domain.h, since it's a better match.

It is possible to turn off the card clock by setting
the frequency to zero via debugfs e.g.
echo 0 > /sys/kernel/debug/mmc0/clock
However that produces an incorrect warning that is
designed to warn if the frequency is below the minimum
operating frequency. So correct the warning.

As of 528bc7808f4e ("mmc: atmel-mci: Release mmc resources on failure in probe"),
the atmci_probe() function calls atmci_cleanup_slot in the failure path.
This causes a new warning whenever the driver is built:
WARNING: drivers/mmc/host/built-in.o(.init.text+0xa04): Section mismatch in reference from the function atmci_probe() to the function .exit.text:atmci_cleanup_slot()
The function __init atmci_probe() references
a function __exit atmci_cleanup_slot().
Gcc correctly warns about this function getting dropped in the link stage
for the built-in case, which would cause undefined behavior when this error
path is hit. The solution is to simply drop the __exit annotation.

Add MMC_POWER_UNDEFINED for power_mode in struct mmc_ios and use it as
the initial value of host->ios.power_mode.
For hosts with MMC_CAP2_NO_PRESCAN_POWERUP, this makes the later
mmc_power_off() do real power-off things instead of NOP, and further
prevents state messed up in cards that was already initialized (eg. by
BIOS of UEFI driver).

This change uses managed resource APIs to allocate resources such as,
clk, gpio, io in order to simplify the driver unload or failure cases.
Hence does away with release statements of the same resources in error
labels and remove function.

If we're tuning on a big-endian CPU we'll never determine we properly
tuned the device because we compare the data we received from the
controller with a table that assumes the CPU is little-endian.
Change the table to be an array of bytes instead of 32-bit words
so we can use memcmp() without needing to byte-swap every word
depending on the endianess of the CPU.

As soon as the CD IRQ is requested, it can trigger, since it's an
externally controlled event. If it does, delayed_work host->detect will
be scheduled.
Many host controller probe()s are roughly structured as:

In 3.17, CD IRQs can are enabled quite early via *_probe() ->
mmc_of_parse() -> mmc_gpio_request_cd() -> mmc_gpiod_request_cd_irq().
Note that in linux-next, mmc_of_parse() calls mmc_gpio*d*_request_cd()
rather than mmc_gpio_request_cd(), and mmc_gpio*d*_request_cd() doesn't
call mmc_gpiod_request_cd_irq(). However, this issue still exists if
mmc_gpio_request_cd() is called directly before mmc_start_host().
sdhci_add_host() may fail part way through (e.g. due to deferred
probe for a vmmc regulator), and sdhci_pltfm_free() does nothing to
unrequest the CD IRQ nor cancel the delayed_work. sdhci_pltfm_free() is
coded to assume that if sdhci_add_host() failed, then the delayed_work
cannot (or should not) have been triggered.
This can lead to the following with CONFIG_DEBUG_OBJECTS_* enabled, when
kfree(host) is eventually called inside sdhci_pltfm_free():

The object being complained about is host->detect.
There's no need to request the CD IRQ so early; mmc_start_host() already
requests it. For most SDHCI hosts at least, the typical call path that
does this is: *_probe() -> sdhci_add_host() -> mmc_add_host() ->
mmc_start_host(). Therefore, remove the call to mmc_gpiod_request_cd_irq()
from mmc_gpio_request_cd(). This also matches mmc_gpio*d*_request_cd(),
which already doesn't call mmc_gpiod_request_cd_irq().
However, some host controller drivers call mmc_gpio_request_cd() after
mmc_start_host() has already been called, and assume that this will also
call mmc_gpiod_request_cd_irq(). Update those drivers to explicitly call
mmc_gpiod_request_cd_irq() themselves. Ideally, these drivers should be
modified to move their call to mmc_gpio_request_cd() before their call
to mmc_add_host(). However that's too large a change for stable.
This solves the problem (eliminates the kernel error message above),
since it guarantees that the IRQ can't trigger before mmc_start_host()
is called.
The critical point here is that once sdhci_add_host() calls
mmc_add_host() -> mmc_start_host(), sdhci_add_host() is coded not to
fail. In other words, if there's a chance that mmc_start_host() may have
been called, and CD IRQs triggered, and the delayed_work scheduled,
sdhci_add_host() won't fail, and so cleanup is no longer via
sdhci_pltfm_free() (which doesn't free the IRQ or cancel the work queue)
but instead must be via sdhci_remove_host(), which calls mmc_remove_host()
-> mmc_stop_host(), which does free the IRQ and cancel the work queue.

Previously only the ACPI PM domain was supported by the sdio bus.
Let's convert to the common attach/detach functions for PM domains,
which currently means we are extending the support to include the
generic PM domain as well.
